In addition, I am not currently under investigation or on probation for such offenses, either by a law enforcement agency, the University of Michigan, or any other educational institution. I hereby give my permission to the University of Michigan Department of Public Safety and Security to conduct a security check on me. Contingent on the results of the security check and interview process, SAPAC reserves the right to reject my application. I understand that the information will only be used for the purpose of evaluating my eligibility to be a volunteer for SAPAC. SAPAC cannot be used as a method of fulfilling court ordered or university judicial sentencing community service.
