Safety Analysis of the Incident

Educators: For information about accessing the solutions to the safety modules, please contact Professor H. Scott Fogler via email at sfogler@umich.edu

The Chemical Safety Board (CSB) has documented and made videos of a number of accidents that have occurred over the last 40 years. These videos and associated modules can be found on the safety website (http://umich.edu/~safeche/). For each safety module you are asked 1) to view the CSB video, and 2) to fill out the safety algorithm and 3) then make a few calculations specific to the module. The Safety Analysis of the Incident you are to use in analyzing the accident after viewing the video is shown below.

The safety algorithm is applied to an incident involving a tablespoon of acetone in a welding accident that was caused a fatality is shown on the safety module. (http://umich.edu/~safeche/assets/pdf/courses/Problems/230Materials&EnergyModule(2)PS060818-WeldingDrum.pdf)
